How QOF income is calculated: Each QOF point your practice earns is worth a cash amount that depends on your practice list size and prevalence. NHS England publish the exact value after year-end. This tool uses an estimated value — the national average for 2026/27 is approximately £213 per point for an average-sized practice. You can adjust this if you have a more accurate figure from your ICB.

2026/27 changes to QOF: New indicators have been added for diabetes (8 NICE care processes, gestational diabetes), obesity (referral to weight management, pharmacotherapy), and heart failure (4-pillar therapy for HFrEF). The AF upper threshold has increased to 95%. Asthma now includes patients aged 5 and over. Indicators marked New or Changed reflect these updates.

How the calculation works: Each indicator has a lower threshold (below which you earn no points) and an upper threshold (at which you earn full points). Your points are scaled proportionally between the two. For example, if an indicator has thresholds of 40–80% and you achieve 60%, you earn 50% of the available points.

About this calculator

The Quality and Outcomes Framework (QOF) is the voluntary annual incentive programme for GP practices in England, rewarding achievement across clinical, public health, and population health indicators. This free estimator helps practice managers, GPs, and PCN teams forecast their QOF income for 2025/26 before the end-of-year reconciliation.

Enter your practice’s expected point achievement by domain and your registered list size to calculate your estimated QOF payment. The calculator applies the current national point value and Adjusted Population Prevalence (APCA) methodology used by NHS England.
